Osteoporosis Program

The program is divided into 5 sessions. Each session offers specific information about all aspects of bone health. The sessions are as follows:
  1. Bone Basics.  Presented by: Nurse Practitioner
  2. Nutrition for Osteoporosis.  Presented by: Registered Dietitian
  3. Medication and Treatments for Osteoporosis.  Presented by: Pharmacist
  4. Fall Prevention: Protecting your Bones.  Presented by: Health Promoter
  5. Exercise and Movement.  Presented by Health Promoter
         
The Aim of the program
Improve patient knowledge of osteoporosis and Osteopenia, modifiable risk factors, signs and symptoms, benefits of early diagnosis and options for treatment
The Goals of the program
  • Improved prevention of osteoporosis fractures
  • Improved bone health knowledge

Population of Interest
This program is designed for clients with osteoporosis or osteopenia, or those with risk factors for developing osteoporosis or osteopenia or those with an interest in the subject who are rostered to the Timmins Family Health Team.
Program Strategies and Process/Activity Indicators
The strategies that will be used to help achieve the program goals and objectives will include: Communication, Education, Organizational Development, Policy Change and Advocacy
