Our Health Team

The TAFHT brings together different health care professionals to co-ordinate quality care for you - the patient. Our health team has the following health care professionals;

Dietitian : The dietitian services are available to people of all ages with a referral from your health care provider. Registered dietitians are the recognized food and nutrition professionals in Canada.

Together you and the dietitian can develop a plan to help you reach your goals through individual treatments and/or education programs according to need. The dietitian works at the Administration Site.

Geriatric Nurse:  The geriatric nurse works with local community services to provide clients with care coordination, caregiver support and most importantly client centered care. This program will allow clients to remain as independent and healthy as possible, with emphasis on providing on going support for the caregivers. The Geriatric Nurse also coordinates the Geriatric Dementia Program. The geriatric nurse works at the Third Ave Site.

Health Promoter The health promoter works with the health team and community groups to plan and develop programs that help people improve and manage their health. The health promoter offers group sessions on a variety of healthy lifestyle topics, develops health promotion resources for health team and clients.  The health promoter works at the Administration Site.

Nurse PractitionerNurse practitioners are registered nurses with additional training and an extended scope of practice so they can provide a full range of health care services to individuals, families and communities. Nurse Practitioners can diagnose and treat illnesses and/or injuries, order and interpret diagnostic tests, write prescriptions, provide treatments and/or procedures and make referrals to family physicians, specialists and other health professionals  The nurse practitioners work at the 101 Mall Site,  Third Avenue Site, Algonquin West Site,  and the Algonquin East Site

Physician: Family physicians are key members of the health care team, and provide the majority of your medical primary care. Our group of physicians work with other health care professionals such as nurse practitioners, nurses, social workers, dietitian and pharmacist to offer additional programs and services aimed at providing care that is more accessible and comprehensive. Registered Nurse and Registered Practical Nurse: Nurses within our team work with our doctors, nurse practitioners, social workers, health promoter, dietitian and pharmacist to provide primary care that promotes health and prevents disease. All of our sites nurses provide care with specialized programs for Well Babies/Well Child, Hypertension and Anticoagulation (INR) therapy. Contact us

Social Workers : Social work services are available to people of all ages with a referral from your health care provider. Services include counsellor-led groups for problems such as depression, anxiety, grief, self-esteem difficulties, stress management and chronic pain. The social workers work at the Administration Site.

Child Psychologist: Child Psychology Services are available to children ages 6 to 16 with a referral from your health care provider. Services include psychological and psychoeducational assessments for learning disorders, attention deficit hyperactivity disorder, autism spectrum disorder, giftedness, anxiety, and other psychological or behavioural issues. The child psychologist works at the 101 Mall Site.
