Advance Care Planning
Advance Care Planning is a process of thinking about and sharing your wishes for future health care. It can help you tell others what would be important if you were ill and unable to communicate. It’s a way to give loved ones the confidence to make decisions for you during a difficult time. While you can choose whether or not to record your wishes, Advance care planning is a way to give those who will be required to provide consent (or refusal of consent) for your medical treatment and care the confidence to make decisions on your behalf when you are mentally incapable to do that for yourself.
